Addressing Major Hazard Installation (MHI) Compliance Gaps: From Findings to Practical Implementation

In this CAIA (Chemical and Allied Industries’ Association) webinar recording, regulators, an Approved Inspection Authority and industry specialists unpacked the key findings from recent Department of Employment and Labour Major Hazard Installation (MHI) workshops and inspections. Held on 29 July 2026, the session delivered practical guidance to close compliance gaps, strengthen process safety systems, and align industry practice with regulatory expectations. Participants gained clarity on MHI compliance requirements while the discussion fostered closer alignment between industry, regulators and inspection authorities.

  • The MHI regulatory framework and expectations — Rachel Aphane (Department of Employment and Labour, MHI Specialist) outlined the regulatory landscape and what the Department expects from MHI duty holders.
  • QRA methodologies aligned to SANS 1461 — Samantha Kachikira (ISHECON, Approved Inspection Authority) explained how quantitative risk assessments should be performed and where they commonly fall short.
  • Process safety failures and gaps — Motlatsi Mabaso (CEO, MMRisk) drew on real inspection findings to highlight the process safety failures that most often trigger non-compliance.
